  • Child Participant Registration Instructions

    C.A.S.T. for Kids events are designed for children with special needs. Children must be accompanied by their parent/guardian for the ENTIRE event.  EACH registered child fishing participant must have their own parent/guardian on site. 

    To register a child with special needs, a parent or guardian must complete the form below. You may register up to 4 children with special needs from the same household on this form. No third-party registrations are allowed.

    Once submitted, you will receive a confirmation email. Check your spam folder if your confirmation is not seen in your inbox. The C.A.S.T. for Kids Foundation emails and texts event details and reminders within 2 weeks of event date. If you register the week of the event, you will need to contact the Event Coordinator for event details. Registrations can be canceled by emailing the Event Coordinator.

  • Agreements

  • For each of the agreements below, please read the terms, check the box to mark your acknowledgment, and sign below. Parent/Guardian Signatory must be 18 years of age or older.

    C.A.S.T. for Kids- Agreement

    Please be aware that each participant must be accompanied by a parent/guardian for the entire event for their child’s safety. Only the registered participant and one parent/guardian are guaranteed to go boating at boating events.

  • C.A.S.T. for Kids- Collection of Health Data

    C.A.S.T. for Kids Foundation collects health data about a participant’s disability to provide reasonable accommodations at our C.A.S.T. for Kids events. This information is shared with our Volunteer Event Coordinator in charge of planning this event and any volunteer paired with your child.

  • C.A.S.T. for Kids- Personal Flotation Device Policy

    For the safety of the children, all children (participants, siblings of participants, children of volunteers, etc) must wear a lifejacket at all times when on a dock or on a boat. Life jackets will be provided for your child to use at the event. There is no lifeguard at the event.

  • C.A.S.T. for Kids- Standards of Conduct Policy

    I acknowledge that I have registered as a non-volunteer participant and have been approved by the event coordinator to participate in that role only. I acknowledge that engaging in a role that I have not been approved for is a violation of C.A.S.T. for Kids Foundation’s standards of conduct and is grounds for removal from the C.A.S.T. for Kids event. Further, I agree to conduct myself in accordance with the following C.A.S.T. for Kids Foundation minimum standards of conduct: I agree to follow the instructions of any C.A.S.T. for Kids Foundation employee or representative; I will refrain from using harsh or foul language; I will keep physical touch to the standard of shoulder hugs and high fives; I agree to not to be alone with a child during the event unless I am the parent or guardian of that child; I will refrain from using all alcohol, tobacco, and controlled substances at the event, including during any awards ceremony or similar event related activity; If I take photos of children, I will ask parents before posting those photos on social media or in any other medium and will not post any photos without permission; I will follow the reasonable instructions of any C.A.S.T. for Kids Foundation employee or representative; and I understand if I do not conduct myself to an appropriate standard for a family event as determined by the C.A.S.T. for Kids Foundation in its sole discretion, I will be required to leave the event premises and may be banned from future C.A.S.T. for Kids Foundation events. I agree that I will not post or otherwise send using social media or any similar platform any content that reflects poorly on the C.A.S.T. for Kids Foundation in any posting or other communication that references or includes content related to the C.A.S.T. for Kids Foundation or the event, including but not limited to, content that parents, sponsors, or the C.A.S.T. for Kids Foundation could reasonably find offensive (e.g. racist, sexist, or other discriminatory comments, obscenity, sexual comments), content that is confrontational to or reflects negatively on any C.A.S.T. for Kids Foundation employee or volunteer, or any otherwise controversial content.

  • C.A.S.T. for Kids- Photograph & Video Release and License

    For any photo or video filmed during this C.A.S.T. for Kids Foundation event, I hereby grant to the C.A.S.T. for Kids Foundation on behalf of myself and any minor for which I am the parent or guardian, the irrevocable, perpetual, assignable, and unrestricted license to the rights of my (and/or my accompanying child’s) image, likeness and voice as recorded on audio, video, or other media without payment or any other consideration. I understand that our images may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein our likeness appears. Additionally, I specifically waive any right to royalties or other compensation arising or related to the use of our images or recordings. I also understand that this material may be used in diverse promotional and/or educational settings within an unrestricted geographic area. I understand this permission signifies that photographic or video recordings of me and/or my child may be electronically displayed via the internet or in any other public setting. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed on behalf of myself and any accompanying child, I hereby specifically release the C.A.S.T. for Kids Foundation and its event sponsors, organizers, and assigns from any and all claims arising out of or related to the use of any photo, video, or other likeness of me or any of my accompanying children in any material or medium for any promotional, educational, or other legitimate purpose of the C.A.S.T. for Kids Foundation and its event sponsors, organizers, and assigns.

  • C.A.S.T. for Kids- General Release; Indemnification

    I, the undersigned, on behalf of myself and any child whom I am the parent or guardian of, give my permission for myself (and, as applicable, any child whom I am the parent or guardian of) to attend and participate in the C.A.S.T. for Kids Foundation event. This C.A.S.T. for Kids Foundation event may be a test of a person’s physical and mental limits and carries with it the potential for death, serious injury, and property loss, despite our, your, or others’ implementation of all reasonable precautions. The risks include but are not limited to: dehydration; allergic reaction; knee, back, head, eye, or foot injuries; cuts and punctures, including from fish hooks; muscle strains, pulls or tears; abnormal blood pressure; fainting; heart rhythm disorders or heart attack; stroke; or even death; actions of other people including, but not limited to, participants, volunteers, spectators, coaches, event officials, event monitors and/or producers of the event; inclement weather and/or other hazardous natural conditions; and contracting illness (including COVID-19). I further acknowledge that certain risks may result from or be compounded by the actions, omissions, or negligence of C.A.S.T. for Kids Foundation employees, C.A.S.T. for Kids Foundation volunteers, or others, including any negligent emergency response or rescue operations of the C.A.S.T. for Kids Foundation. I hereby assume on behalf of myself and any child who I am the parent or guardian of all of the risks of participating in this event, and I warrant that I am the guardian of any minors that I bring to the event without a parent or other legal guardian also attending the event. I state to the best of my knowledge that I and my accompanying child have no medical, physical, mental, or emotional health conditions that would hinder our active participation in this program or put other participants at heightened risk of illness, and I recognize the C.A.S.T. for Kids Foundation cannot accommodate special medical related requests, including but not limited to special requests related to food allergies. In the case of an emergency in which I am unable to give permission for medical treatment, I authorize the staff, sponsors, or agents of the C.A.S.T. for Kids Foundation, and all event volunteers, to obtain whatever medical treatment they deem necessary for my and/or my accompanying child’s welfare. I further understand and agree that I will be financially responsible for all medical provider charges and fees incurred in the rendering of said emergency medical treatment, regardless of whether my medical insurance would cover such charges and fees. With full acknowledgement of all risks, I hereby assume full responsibility for all risk of injury, death, or other loss that may result from my and/or my accompanying child’s participation in this activity and hereby agree not to sue and to hold harmless, release, and forever discharge, the C.A.S.T. for Kids Foundation and all its employees, directors, officers, agents, partners, sponsors, vendors, and volunteers related to this event from any and all claims and demands whatsoever. This waiver of liability will be construed broadly to provide a release and waiver of claims against the C.A.S.T. for Kids Foundation and all its employees, directors, officers, agents, partners, sponsors, vendors, and volunteers to the maximum extent permissible under applicable law. Further, I agree to indemnify, defend, and hold the C.A.S.T. for Kids Foundation harmless against any claims, damages, fines, violations, or actions brought by a third-party arising from or relating to my actions during the event.

    If for any reason any provision of this agreement is held to be invalid or unenforceable to any extent, then (A) the provision will be interpreted or reformed to the extent reasonably required to render the provision enforceable and consistent with the original intent underlying the provision; (B) the provision will remain in effect to the extent that it is not unenforceable; and (C) the unenforceability of the provision will not affect any other portion of this agreement.

  • Electronic Signature

  • Electronic Signature Agreement: Signatory must be 18 years of age or older, and agrees that this Agreement may be executed and delivered by electronic signatures and that the signatures appearing on this Agreement are the same as handwritten signatures for the purposes of validity, enforceability and admissibility.
